How to Treat Agoraphobia — Without Being Forced Outside

The standard treatment for agoraphobia is graded exposure. There is another way — one that addresses the panic, not the place.

If you have agoraphobia, the standard treatment offered by the NHS will be graded exposure: a programme of returning to feared places in small steps until your brain learns they are safe.

For some, it works. For many, it stalls at the first significant step — because being asked to walk into the very place that triggers panic, while panic-prone, is a near-impossible ask.

There is another way.

Treat the panic, not the place

Agoraphobia is downstream of panic disorder. The fear is not really of supermarkets, motorways or queues — it is of having a panic attack while in them. Address the panic at its root, and the fear of the location disappears with it.

This is the principle the LAR programme is built on. We start at the source: the sensitised anxiety response that generates panic in the first place. As that response normalises, panic attacks become rarer, then absent — and travel resumes without forced exposure.

Practical steps in the LAR programme

1. Education: understanding exactly what is happening physiologically, which alone reduces the fear of fear

2. Behavioural guidance: specific actions and non-actions in everyday life that allow the anxiety response to de-sensitise

3. Coach-led sessions: weekly Zoom, Phone or FaceTime calls with a recovered LAR Coach who can spot exactly where you are getting stuck

4. Gradual return: as panic recedes, the world re-opens naturally — at the pace your nervous system can sustain

Realistic timeframes

Most LAR clients with agoraphobia report meaningful change within weeks and substantial recovery within months. Cases of housebound agoraphobia of 10+ years have resolved fully under coach-led guidance.

A note on medication

LAR Coaching is fully compatible with any medication you are currently on. We work alongside your GP. Many clients eventually choose, with prescriber support, to taper — but this is never asked of you.
